Many procedures that a health plan initially denies coverage fpr are eventually paid as a result of successful benefit claim litigation.  Sometimes, obtaining coverage is as easy as appealing an initial denial within the plan's internal appeals process.  However, sometimes denials will not be overturned unless litigation is pursued.
